Your scheduled appointment time has been reserved specifically for you. We request a 24-hour notice if you need to cancel your appointment. We require a 7-day notice for cancellation of surgical appointments.
Financial Policy & Insurance Information
Our office is committed to helping you make the most of your dental insurance benefits.
Because insurance policies vary, we can only provide an estimate of coverage in good faith. Final payment decisions are determined by your insurance company
Patient Responsibility: Your estimated portion is due at the time of service
Insurance Billing: As a courtesy, we will submit all claims to your insurance provider. We allow 45 days for insurance payment. If payment has not been received within 60 days, the balance becomes your responsibility and is due in full
Treatment Plan & Costs: Before beginning treatment, we will provide you with a treatment plan and an estimate of costs. We will review these details together so you understand your financial responsibility
Payment Options: For your convenience, we accept Visa, MasterCard, and Discover. We also offer financing options through CareCredit. If you require special arrangements, please speak with our office manager prior to treatment
Follow-Up on Claims: While we will do everything we can to help you receive your full insurance benefits, you are ultimately responsible for your account. We encourage you to contact your insurance provider if your claim is not processed within 30 days.
